Take time to review the attached qualification standards for Nurse II.   In bullet or paragraph form, please provide information addressing/justifying each of the 9 criteria as they pertain to your nursing practice.  Include specific examples of actions you have taken in your practice (i.e., problem identification, interventions, outcomes), as appropriate.   This document should be titled, “Addendum.”  An Addendum example (see attachment)  has also been provided to guide you through the process.  You may include anything else you want the Board to know about your clinical practice that you believe may positively impact your salary determination.  I will be presenting this information to the NPSB on your behalf, so additional information regarding the above criteria will allow us to present the strongest possible package.  Please note that the NPSB is the only authority that can determine your nursing level.

For your Addendum, also include, in bullet form, anything you want the Board to know about your nursing career that you feel is noteworthy.  Your file can only be presented once and after it is presented, the Board will not accept any additional information.  Be your own best advocate and acknowledge your accomplishments.  

Per our discussion, include in your Addendum that you have served as noted below: 

1. Charge Nurse
2. Preceptor
3. Superuser (state what you are super user of)
4. Skin Care Champion
5. Peer Review Process (brief statement of things you did to review the work of your peers, such as chart audits, etc.)

Also: 

1. Copy of your performance evaluation (any that have taken place within the last 2 years); if possible.  If your rating is above satisfactory, you will be awarded additional points toward your starting salary.
